MV Cubs Club Weekly Signup Form

MV Cubs Club Weekly Signups

This form is for the weekly signups in our Cubs Club before and after school program. Leading Edge Academy will be billing for the service according to this form. The payments made for the Cubs Club are Non-Refundable. Please double check with the school calendar for special dates like half days or holidays. A different form will be available for that week on our school stores page: www.leadingedgeacademy.com/school-stores/ This form will be posted at the beginning of the month those schedule changes would occur.

*A one time registration fee will be required per student participating, if you have not taken care of it already please make sure to let us know by indicating it in the requested fields*

**We recommend setting up the desired days and service(Before School, After School or both) as early as possible. If we have less than 24hrs notice for a student to participate in our before or after school program we will have to charge a higher Drop In fee for that day.**

Weekly Detail for Cubs Club Before or After School program

Please complete the following information for the week you will be billed for.

You can schedule Cubs Club by the day. Just select the times you need the Cubs Club service(AM, PM or both) for each day(Mon-Fri) of the week. Make sure to select the correct number of students participating each day as well. Please keep in mind that some days may be Early Dismissal(12pm) or no school, these dates will not correct automatically so it will be YOUR responsibility to make sure the schedule is correct. Double check on the school calendar for this year(2024-25) if you are unsure when those days are.

The required portions of the form must include a response in order for a user to submit the form.

*If you are setting up to be invoiced for drop in fee only please select No cubs club for each day(Mon-Fri) below before proceeding to complete that section*

**If you have a drop in during the week you are scheduling for please select No Cubs Club for the day of drop in then complete the section under Drop in for Cubs Club**

***Example: if it is Tuesday morning and you are doing drop in for your student that day select no cubs club under Monday and Tuesday-then you can fill in the rest of the week as needed. You will set Tuesdays date, what time of day(Am, PM, both) for drop in and how many students***

If you could not do advance(24 hours +) notice of the students participation in our Cubs Club Before and After School activities. Then you will need to enter that below. Let us know: 1. The day for drop in to the program. 2. The number of students doing cubs club for that day. 3. Was it the morning, afternoon or both times of the day for drop in to Cubs Club program? You can do up to 5 days at a time for cubs club drop in if you need that many. Just make sure to check yes for "do you need to add another day for Cubs Club Drop In?"

If Students in your group are not all doing Afternoon Cubs Club(because of other school Clubs, Sports or activity) and still want to receive your multi-child discount we can help on that. You can Adjust your fees here for students not participating in the afternoon Clubs for your family. Just let us know how many students are not doing the afternoon Cubs Club and how many days they are not participating for that week. Our form will adjust the price accordingly for your invoice. *PLEASE NOTE THIS IS ONLY FOR REGULAR(MON-THURS) FEES, IT SHOULD NOT BE USED WITH DROP INS OR ON FRIDAYS.
